Description

Parenteral verabreichbare , unter Hitzesterilisationsbedingungen stabile O/W-Emulsion eines Röntgenkontrastmittels Parenterally administrable O / W emulsion of an X-ray contrast medium that is stable under heat sterilization conditions
The invention relates to a parenterally administrable O / W emulsion of an X-ray contrast agent which is stable under heat sterilization conditions and is based on iodinated fatty acid esters, which in a suitable concentration, if appropriate after appropriate dilution, is advantageous in X-ray diagnostics, for example in lymphography , can be used .
From DE 41 11 939 C2 a parenterally administrable, heat-sterilizable O / W emulsion of an X-ray contrast medium is already known, in which the inner phase contains one or more oil-soluble organic iodine compounds and the outer phase has distilled water with isotonizing additives. By adding a physiologically compatible buffer system, which is composed of sodium hydroxide solution and disodium hydrogenphosphate and / or disodium glycerophosphates, it is possible to stabilize the emulsion in such a way that it can be sterilized in an autoclave at 121 ° C. for 15 to 20 minutes without the Structure of the emulsion is adversely affected. The finished emulsion then has an average droplet diameter of the inner phase of at least 0.6 μm. Reference is expressly made to the prior art discussed in detail in the introduction to DE 41 11 939 C2. Although the known emulsions described above have already proven their worth in X-ray diagnostics, it has been shown in practice that even safer handling of the O / W emulsions containing the X-ray contrast medium is desirable, particularly during and after the sterilization phase. This presupposes that the emulsions can be made even more stable with respect to the thermal load at the sterilization temperatures of 120-121 ° C., so that operational fluctuations in the sterilization conditions, as they occur in daily operation, do not affect the product characteristics to have an undesirable effect. This would make the production of such O / W emulsions in the required quality even less critical and easier to carry out.
The present invention is therefore based on the object of further improving the stability of the X-ray contrast medium emulsions, in particular with regard to heat sterilization at 120 to 121 ° C. in an autoclave, and thereby ensuring that the internal phase of such an O / W Emulsion which has iodinated fatty acid ester droplets with an optimally adjustable average droplet diameter for very good X-ray absorption. In addition, such a parenterally administrable emulsion should be well tolerated by the patient and, under suitable storage conditions, should have very good storage stability.
__ Überraschenderweise hat sich ergeben , daß das erfindungsgemäß vorgeschlagene Puffersystem die in Rede stehende O/W-Emulsion einesThis object is achieved according to the invention by a parenterally administrable O / W emulsion of an X-ray contrast medium which is stable under heat sterilization conditions and has an inner phase composed of one or more suitable oil-soluble iodinated fatty acid esters, optionally together with one or more highly refined glyceride oils, one or several emulsifiers and optionally co-emulsifiers and an outer phase of distilled water with isotonic additives and a physiologically compatible buffer. This emulsion according to the invention is characterized in that the physiologically compatible buffer consists of 0 to 12 mol / l sodium hydroxide solution and 2 to 10 mol / 1 sodium carbonate and / or sodium hydrogen carbonate. nat, based in each case on the volume of the finished emulsion, and that the iodinated fatty acid esters are present in the inner phase with a selectively adjustable average particle size in the range from 0.2 to 4.0 μm. __ Surprisingly, it has been found that the buffer system proposed according to the invention is the O / W emulsion in question
Röntgenkontrastmittels nicht nur derart stabilisiert , daß diese eineX-ray contrast media not only stabilized in such a way that this one
Die erfin- dungsgemäße O/W-Röntgenkontrastmittel-Emulsion besitzt außerdem unter Stickstoffatmosphäre eine ausgezeichnete Lagerstabilität und weist eine sehr gute Absorption von Röntgenstrahlen auf , so daß sie in der Röntgendiagnostik vorteilhaft eingesetzt werden kann .Heat sterilization at 121 ° C in an autoclave survives completely without problems for 20 minutes, but moreover allows the average particle size of the in the inner phase of the O / W by suitable choice of the ratio of carbonate buffer to sodium hydroxide solution, total concentration of the buffer system and the duration of the heat sterilization To influence the emulsion of iodinated fatty acid esters in a targeted manner so that, depending on the selected conditions, the average particle size of the iodinated fatty acid esters after sterilization remains unchanged or is increased to a predeterminable extent compared to that prior to sterilization. This is clearly demonstrated using the examples described below. This enables the X-ray contrast agent emulsions to be better matched to the respective special requirements of the examination methods and / or. to adapt to the special wishes of a user. This is not possible or only possible to a limited extent with the X-ray contrast medium emulsions known to date. The O / W x-ray contrast medium emulsion according to the invention also has excellent storage stability under a nitrogen atmosphere and has very good absorption of x-rays, so that it can be used advantageously in x-ray diagnostics.
Die jodierten Fettsäureester werden gewöhnlich in einer Menge von 5 bis 40 Gew.-7o, bezogen auf 5die fertige Emulsion, eingesetzt.X-ray contrast agents containing iodine are used in the O / W emulsions according to the invention which are known per se and are commercially available, parenterally administrable oil-soluble organic iodine compounds which provide sufficiently strong positive contrasts in the X-ray imaging of organs, namely iodinated fatty acid esters, in particular fatty acid glycerides all fatty acid triglycerides such as se fatty acid ethyl ester of iodized poppy oil, which is available under the trade name Lipiodol UF, also iodized soybean oil, cottonseed oil, peanut oil, iodized fish oils and the like. The iodinated fatty acid esters are usually used in an amount of 5 to 40% by weight, based on the finished emulsion.
The emulsion according to the invention can optionally also contain one or more non-iodinated, highly refined glyceride oils which can be used in the parenteral nutrition field, such as e.g. Soybean oil, safflower oil, medium-chain triglycerides and the like. By adding them, the density of the oil droplets as the inner phase in the emulsion can be reduced, thereby preventing sedimentation.
The emulsion according to the invention also contains one or more 15 emulsifiers which must have the purity required for parenteral use. Such emulsifiers are known and commercially available. Naturally occurring emulsifiers are preferably used for the purpose according to the invention, in particular lecithins, for example from soybeans, particularly preferably egg lecithins. Suitable ratios of the phospholipid fractions present in the lecithins can be adjusted by special refining and fractionation processes known per se. Very particularly preferred is the use of egg lecithins with a phosphatidylcholine content> * 757 0 and a cephalin content - * 157o. It is also possible to use high-purity fractions of soy lecithin.
Furthermore, the emulsion according to the invention can optionally contain suitable co-emulsifiers, in particular the alkali salts of long-chain fatty acids, such as e.g. palmitic acid, oleic acid or stearic acid are suitable.
The amounts of lecithins are generally in the range from 0.4 to 35 g / l of the finished emulsion according to the invention, the amounts of co-emulsifiers, for example alkali metal salts of fatty acids, generally in the range from 0.2 to 1 g / l finished emulsion. The outer phase of the emulsion according to the invention contains distilled water or. highly purified water of injectable quality (aqua ad injectabilia) and isotonizing additives, with the help of which the external phase of the emulsion according to the invention is adjusted isotonic with human blood. Physiologically compatible polyols such as glycerol, sorbitol or xylitol are particularly suitable as isotonizing additives. Glycerol is particularly preferably used for this. The isotonization additives are added to the emulsion in the amounts necessary for the isotonization, for example 25 g / l in the case of 10 of the glycerol, based on the finished emulsion.
The excellent heat and storage stability of the emulsion is ensured by the addition of a physiologically compatible buffer made of sodium carbonate and / or sodium

The emulsion according to the invention is produced in the manner known in principle from DE-PS 37 22 540, see in particular column 13/14, care being taken during the entire production process and, if appropriate, the subsequent storage of the finished emulsion that the components and obtained mixtures and the finished emulsion are kept constantly under a nitrogen atmosphere. The individual process steps can be briefly described as follows:
First, the required amounts of lecithin are constantly stirred into a corresponding amount of aqua ad injectabilia, which is approx. Is 55 to 60 ° C, entered and then the mixture for a while, z. B. 15 to 20 minutes, further stirring. At the same time, appropriate amounts of glycerol and sodium oleate are added and dissolved with constant stirring in a second corresponding amount of aqua ad injectabilia, which is also heated to 55 to 60 ° C. The solution obtained is then filtered under nitrogen pressure through a suitable membrane filter, for example with a pore size of 0.2 μm, and the filtrate is added to the prepared water / lecithin mixture, the temperature being kept at 55 to 60 ° C. . A measured suitable amount of iodinated fatty acid esters, for example fatty acid ethyl ester of iodinated poppy oil, is heated to 50 to 60 ° C. and passed through a nylon membrane filter with a pore size of e.g. B. 0.2 μm filtered and the filtrate added directly into the prepared aqueous mixture of lecithin, glycerol and sodium oleate with constant stirring, for example using a mechanical high-frequency device (Ultra-Turrax) together with a stirrer, a crude emulsion being formed. After the iodized oil has been completely added, the crude emulsion formed is further emulsified for a while in order to obtain a corresponding crude emulsion. The entire mixture is kept at 55 to 60 ° C and blanketed with nitrogen.
The emulsion obtained is further emulsified in a closed system in a suitable 2-stage homogenizer in a first stage at 400 bar and in a second stage at 100 bar, the temperature being kept between 50 and 60 ° C. After the high-pressure homogenization has been carried out, the emulsion is transferred to a storage tank. There, the emulsion is then diluted to a suitable concentration with the addition of the appropriate amounts of carbonate buffer and sodium hydroxide solution. The emulsion is reduced to approx. Cooled 10 to 15 ° C. The emulsion is then filled under a nitrogen protective atmosphere, which is then followed by heat sterilization in a rotary autoclave at 121 ° C. for 15 to 25 minutes.
Die Proben sind danach über längere Zeit lagerungsstabil .After cooling, the emulsions according to the invention must be protected from the effects of light and oxygen, which is why they are filled under nitrogen. After sterilization, the filled emulsion quantities are stored at +4 and at 21 ° C. The samples are then stable in storage for a long time.
Die Erfindung wird anhand der nachfolgenden Beispiele weiter erläu¬ tert : Beispiel 1The invention is further illustrated by the following examples: example 1
An O / W emulsion of an X-ray contrast medium according to the present invention was produced as follows:
Danach wurden unter Fortsetzung der Stickstoffbegasung 16 g Eilecithin in ca . 2 Minuten231 ml of distilled water for injection was placed in a 5 nitrogen gas container and heated to 55-60 ° C and held at this temperature during the subsequent process steps. The water was gassed with nitrogen until the oxygen content had dropped below 0.1 mg / l. Then 16 g egg lecithin in approx. 2 minutes

Die erhaltene , 55 bis15 of the other process stages kept at this temperature. The water was again gassed with nitrogen until the oxygen content had dropped below 0.1 mg / l. Then 25 g of glycerol (1007%) and 0.3 g of sodium oleate were added to the water and slowly dissolved with stirring. The received, 55 to

Anschließend wurde auf die Emulsion umgeschaltet .200 g of a fatty acid ethyl ester of iodinated poppy oil (Lipiodol UF) 25 were heated to 50 to 60 ° C in a container under nitrogen gas and through a nylon membrane filter with a pore size of 0.2 microns within 20 to 25 minutes directly into the prepared aqueous Mixture of lecithin, glycerol and sodium oleate added with constant stirring, the mixture being treated simultaneously by a mechanical high-frequency device (Ultra-Turrax) with the open generator (G 6) and coarse generator (G 2) running. After the X-ray contrast medium had been completely added, the crude emulsion formed was further emulsified for 25 minutes. During the production of the crude emulsion, it was kept at a temperature in the range 35 from 55 to 65 ° C. and was continuously blanketed with nitrogen. The raw emulsion was then passed through a membrane filter with an average pore size of 40 μm under a nitrogen pressure of approx. 0.5 bar placed in a 2-stage homogenizer suitable for the production of fat emulsions and homogenized there in the first stage at 400 bar and in the second stage at 100 bar. The required homogenization pressure was achieved with a hot distillate through a by-pass. It was then switched to the emulsion.
During the homogenization the temperature was approx. 60 ° C. The emulsion formed was placed in a storage tank overlaid with nitrogen. Here the emulsion was allowed to rest with occasional slow stirring. The emulsion was then subjected to two further homogenization steps, the temperature again being kept at about 60.degree. About 500 ml of this emulsion were cooled to 12 ° C. and placed in a receiver from 500 ml of a solution of 10 mmol / 1 Na 2 CO “in oxygen-free distilled water, likewise cooled to 12 ° C., so that the mixture was diluted 1: 1 Emulsion contained 5 mmol / 1 Na ^ CO «. During the run-in of the homogenized emulsion into the receiver, no further gassing with nitrogen was carried out, but only with nitrogen. With occasional slow stirring, the emulsion was further cooled to 8 to 9 ° C. After this temperature had been reached, the stirrer was switched off.
Der pH-Wert der Emulsion wurde geprüft . Bei einem pH-Wert niedriger als der Sollwert von mindestens pH 10, 0 wurde in den folgendenThe pH of the emulsion was checked. At a pH lower than the set point of at least pH 10.0 was used in the following
Beispielen der pH-Wert durch Zugabe einer entsprechenden MengeExamples of pH by adding an appropriate amount
1-normaler Natronlauge korrigiert . In the present example, such an addition was not necessary because after the addition of the carbonate buffer before the sterilization, the emulsion had a pH of 10.2.
The emulsion obtained was stored in a cooling tank under a nitrogen atmosphere and, before filling, was filtered through a membrane filter with a pore size of 2 to 8 μm, the filling pressure was a maximum of 0.5 bar. The filling was carried out under nitrogen protective gas in glass infusion bottles, with nitrogen being gassed during the filling so that the oxygen content in the glass bottles was less than 0.1 mg / l. After filling, the bottles were closed and crimped with hollow stoppers.
The emulsions according to the invention were protected from light and oxygen during production and storage.
The emulsion filled into the glass infusion bottles was then heat-sterilized in a rotary autoclave at 121 ° C. for 15 minutes. In addition to this holding time, at which the temperature of 12l ° C in the sample is kept constant (actual sterilization phase), the heating-up time until the prescribed sterilization temperature is reached and the fall time, i.e. the cooling phase, still make a certain contribution to the sterilization of the sample, which is also recorded by the so-called F value (in minutes), the F values are given here and in the following examples instead of the holding times. In the present example, the F value was 17.5 minutes.
After sterilization, the emulsion had a pH of 7.68. The optical examination showed that the emulsion obtained met the usual criteria of a parenterally applicable O / W emulsion both before and after sterilization. The average particle size of the emulsion after sterilization, determined with an Automator 2c, was 239.8 nm.
After sterilization, the filled emulsion was protected from light and stored at + 4 ° and at 21 ° C. Random samples showed that the emulsion remained in working order even after a long storage period. Noteworthy with this result is not only that the emulsion provided with the carbonate buffer has very good stability under the heat sterilization conditions used, but that the average particle size of the emulsion droplets after sterilization is 239.8 nm from the average particle size of the emulsion droplets ¬ Chen deviates only slightly before the sterilization, which was 234.8 nm. This opens up the possibility of sterilizing the O / W X-ray contrast agent emulsions according to the invention under suitable conditions without having to accept a substantial coarsening of the iodinated fatty acid ester droplets.

In the following examples, O / W X-ray contrast medium emulsions of the same composition and the same type of preparation as described in Example 1 were used, with the difference that in Example 2 an emulsion without a buffer system and 3 to 6 emulsions with different amounts of the buffer system were used in Examples. The carbonate buffers were added to the emulsions after the 4th homogenization step and the cooling of the emulsions to approx. Added 12 ° C in the following amounts:

In der nachfolgenden tabellarischen Zusammenstellung sind die F o -Werte als Maß für dieThe emulsions obtained all proved to be in order before the sterilization in the optical test. In the following table, the F o values are a measure of the
Dauer der Hitzesterilisation , die durchschnittliche Teilchengröße , einmal gemessen über dynamische Lichtstreuung (Autosizer 2c , Firma Malver.n ) und einmal bestimmt als arithmetisches Mittel aus der mit Hilfe des Coulter-Verfahrens ermittelten Anzahlverteilung der Teilchen¬ größen , sowie für die Beispiele 3 bis 6 die Teilchengrößenverteilung nach der Sterilisation , gemessen nach dem Coulter-Verfahren , ange¬ geben .Duration of the heat sterilization, the average particle size, measured once via dynamic light scattering (Autosizer 2c, company Malver.n) and once determined as an arithmetic mean from the The number distribution of the particle sizes ascertained with the aid of the Coulter method and the particle size distribution after the sterilization, measured according to the Coulter method, for Examples 3 to 6.
The results in the table show very clearly that the emulsion without buffer (example 2) cannot be heat sterilized at 121 ° C. The emulsion is broken after sterilization. In contrast, the emulsions (examples 3 to 6) equipped with the buffer system according to the invention are all fully intact after sterilization. Particularly noteworthy here is the result of Example 6, in which both the average particle size and the particle size distribution in comparison with Example 2 show that the internal phase with respect to the droplet size after sterilization hardly changes compared to the state before sterilization Has . The F values of Examples 4 to 6 also show that these emulsions are extremely stable under the conditions of heat sterilization due to the buffer addition according to the invention. In addition, it can be seen that a relatively small addition of buffer, as in Examples 3 and 4, results in a clear tendency towards particle enlargement after sterilization in the inner phase of the emulsion, i.e. to coarsening of the emulsion, while this tendency decreases with increasing concentration of buffer in the emulsion under otherwise identical conditions.
Entsprechende TabelleAs is known, sodium carbonate dissociates into Na and CO "ions in water and the carbonate ions react with water until a chemical equilibrium is reached to form hydrogen carbonate ions and hydroxyl ions, which results in the extremely alkaline reaction of the sodium carbonate when dissolved in Water results, even if sodium carbonate is used exclusively, hydrogen carbonate ions are present in the buffer solution that forms. It was therefore assumed that a stabilizing effect on the O / W x-ray contrast agent emulsions can also be achieved with sodium bicarbonate instead of sodium carbonate. Appropriate table

Experiments have confirmed this assumption and have shown that the same advantageous stabilizing effect that the carbonate buffer system exerts on the O / W X-ray contrast agent emulsions is also achieved by using sodium bicarbonate instead of sodium carbonate.
